勃起功能障礙與肥胖

肥胖(obesity)
過去認為體態胖胖的是福氣與富裕的象徵,現在的觀念則認為肥胖是一種營養失調的狀況,是屬於現代人的文明病。過重(overweight)或是肥胖可能造成其他疾病或是死亡的認知歷史已超過2000年[1]。根據世界衛生組織的體位定義標準,過重是指body mass index(BMI)在25-29.9 Kg/m2,肥胖是指BMI大於30 Kg/m2,而嚴重肥胖是指BMI大於40 Kg/m2或是大於35 Kg/m2,同時有共病(comorbidity)的情形[2]。

性功能障礙 (sexual dysfunction)
性功能障礙的問題也在目前這繁忙而壓力大的社會中逐漸增加,成為許多男女之間的問題,甚至關係緊張惡化。男性性功能障礙包括勃起功能障礙(erectile dysfunction, ED)、性慾減退(dimished libido)和射精障礙(Abnormal ejaculation)。根據台灣性功能障礙諮詢既訓練委員會的資料顯示,40到70歲的台灣男性有超過五成的比例有性功能障礙的問題[3]。也就是說至少超過一百萬的台灣男性有性功能障礙的問題。再根據1994年Massachusetts Male Aging Study (MMAS)統計,男人到40歲左右,大約就有40%有性功能障礙的問題。而且每增加10歲就增加10%的盛行率[4]。

肥胖與性功能障礙
肥胖的問題影響全球約4億的成人[5]。近期一項研究顯示有腹部肥胖(abdominal obesity)的男性有接近73%有ED的症狀,47%有睪固酮缺乏的症狀(testosterone deficiency syndrome,TDS),有62%有代謝症候群(metabolic syndrome, MetS)的情形[6]。同時發現有ED的男性中79%的BMI是超過25 Kg/m2,而BMI超過30Kg/m2的男性患有ED的風險是正常人的3倍。而內臟肥胖(visceral obesity)會導致人體加劇的發炎反應,因此也會造成內皮細胞的功能破壞(endothelial dysfunction)以及血漿中睪固酮的濃度下降。如此就會造成血管性疾病,例如ED還有性腺功能低下症(hypogonadism)。早期流行病學的研究發現性腺功能低下症是男性老人死亡的一項預測因子[7.8.9]。

肥胖造成ED的機轉
肥胖與MetS是息息相關的。肥胖與心血管疾病的相關是造成MetS與ED一項重要的病理生理因子[10]。根據Corona等人最近的文獻回顧,特別是中央型肥胖(central obesity)與動脈性ED和血漿中睪固酮濃度降低有密切的關聯性[11]。與MetS關連的肥胖,在引起人體各項疾病的慢性發炎中扮演一項重要的致病角色,包括下泌尿症狀和ED。肥胖與低發炎狀況有關,而這發炎的結果造成內臟脂肪組織釋放脂肪細胞激素(adipocytokines)引發內皮細胞功能破壞和胰島素阻抗(insulin resistance)[12]。而脂肪組織細胞也負責調控透過荷爾蒙和神經訊號以及化學信使(chemical messenger)讓人體能量平衡的路徑。這些化學信使包括 tumor necrosis factor-alpha (TNF-

睪固酮濃度低會導致過多的脂肪堆積在人體內,特別是BMI>35的肥胖男性與控制組比較,血漿中有較高的雌激素(estradiol)及較低的睪固酮濃度[15]。體脂肪比例增加與許多與ED有關的代謝改變情形,包括胰島素阻抗、血脂肪異常(dyslipidemia)、氧化壓力(oxidative stress)增加、脂肪過氧化(lipid peroxidation)、以及高血壓。而這些都是MetS的成分因子[16]。

最近的研究發現肥胖者腰圍的大小與睪固酮濃度高低還有ED的比例有顯著的相關性[17]。腰圍超過94公分的男性74.7%有ED的情形,再細分這些肥胖男性的腰圍大小,介於94-101公分者是61%,而當腰圍超過120公分的男性卻有高達87%有ED的情形。同時腰圍超過120公分的男性,睪固酮濃度缺乏的比例高達87.1%。

減重介入對勃起功能障礙改善的研究
從過去減重的研究發現,體重的降低對於肥胖而且有勃起功能障礙的男性可以明顯改善勃起功能IIEF的分數,同時也改善了血壓血脂肪發炎指數(CRP)白細胞介素(Interleukins)血糖和胰島素阻抗[18]。同時有一篇針對過重或是肥胖的糖尿病病人分別隨機分派至試驗組(強力生活飲食介入)和與對照組。對試驗組進行減重、增加活動量的個人與群體介入,對照組只進行糖尿病控制的支持性協助與教育。經過一年的結果,試驗組明顯體重下降(9.9% vs 0.6%),勃起功能也明顯改善[19]。

結語
肥胖造成性功能障礙的詳細機轉或許未完全確立,但明顯和代謝症候群有關性,肥胖造成的性功能障礙男性,經由體重的降低可以明顯改善勃起功能。
