陰莖彎曲 - Chordee

甚麼叫做陰莖彎曲？
　　廣義的陰莖彎曲指培氏病（Peyronie’s Disease）、尿道狹窄、尿道下裂及陰莖受傷造成的彎曲。Chordee通常是用來代表狹義的陰莖彎曲，指先天陰莖向腹側彎曲。原始名稱是法文（French for cord）指陰莖彎曲像彎曲的弓。

　　陰莖彎曲患者陰莖先天短小，尿道或陰莖腹側有一束纖維化的結締組織，造成陰莖彎曲。

症狀：
　　除了陰莖勃起時彎曲之外，陰莖背側常常可見包皮兜帽或類似屋緣（Dorsal hood deformity）的包皮畸形，並且可見不完整的包皮發育。嚴重的陰莖彎曲有時會造成無法站立解尿，因為會向後下方勾射出尿液，尿濕褲子。

診斷：
　　最通常的症狀是患者勃起時陰莖下彎，若在起初的檢查無法確實知道彎曲的情況，則可在手術時用止血帶紮緊陰莖根部，再注射生理食鹽水入陰莖海棉體造成人工陰莖勃起，這時可觀察其陰莖彎曲之程度，這種技術並且可以用來評估手術後陰莖變直的情形。

治療：
　　一般在小孩六個月之後就可手術，若合併有尿道下列也可同時手術矯正。手術目的有三：(1)改善陰莖的外觀、(2)可以站立小便、(3)順利達成性行為。

　　手術方法因陰莖彎曲之嚴重度而有所不同，例如(1)Nesbit Dorsal Plications、(2)手術去除在尿道開口以及龜頭之間纖維化的結締組織、(3)切開或切除陰莖海綿體的腹壁再植皮Incision of the ventral aspect of corpora cavernosa at the point maximal curvature and place a graft in the defect created as the penis straightens、(4) 切開並且轉移陰莖海綿體（Chordee Correction by Corporal Rotation: The Split and Roll Technique.）

結論：
　　陰莖彎曲是陰莖腹側有一束纖維化的結締組織，拉彎陰莖造成彎曲。治療的目的是矯正彎曲不縮短陰莖長度，恢復正常外觀、尿流和性功能。
